# What do hip studies actually tell you?

Research can't promise your result.

## Does less soreness mean the joint rebuilt?

No. Soreness may ease without new tissue growth. A scan can also change while pain stays. Those results don't answer the same question.

Track the change you can feel.

Walking farther is useful. Sleeping longer is useful too. Neither proves that cartilage, the smooth coating on bone ends, grew back. A scan would check tissue change instead.

## Can a knee study answer for your hip?

Knees are studied more often than hips. A result in knees doesn't prove the same hip result. Your hip won't always share the same pain source.

Ask for research about your sore area.

Pain inside the joint differs from tendon soreness. Back pain can spread toward your hip. The provider must first name the likely sore part. Then ask whether hip studies support the offered care.

## What can you fairly expect?

Choose a change you'll notice during normal days. It might be easier walking or better sleep. Pick a time to review that exact task.

You won't know the result beforehand.

Ask the provider how uncertain the result is. Don't forget to ask which choices remain. If walking keeps getting harder, compare surgery directly. Medical studies can guide you, but they can't choose.

Use the same task on several days. One good afternoon can mislead you. Your notes will show whether relief lasts.

The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

A 2026 systematic review and meta-analysis of 28 randomized trials of intra-articular mesenchymal stem cell-based therapies in knee OA found significant improvements in several pain and function measures (delta-VAS MD -1.67; KOOS pain MD 15.37) but NO significant difference in WOMAC, KOOS quality of life or the Lequesne index, and MRI-based WORMS scores were non-significant - indicating no consistent structural benefit. Its own conclusion: these therapies serve a primarily SYMPTOM-modifying rather than STRUCTURE-modifying role, with higher frequencies of local reactions to weigh against the symptomatic benefit.

5. FORWARD, the longest disease-modifying osteoarthritis drug trial reported to date, gave intra-articular sprifermin (a recombinant FGF-18) or placebo to knee OA patients and followed 378 of them for 5 years. Sprifermin produced a significant, sustained dose-response INCREASE in total femorotibial cartilage thickness versus placebo - and WOMAC pain improved about 50% from baseline in ALL groups, including placebo. It is the cleanest demonstration in the literature that adding measurable cartilage and relieving pain are two different results, and that one does not deliver the other.

6. A GRADE-rated systematic review and meta-analysis of 16 randomized trials (807 participants) found that MSC therapy for chronic knee OA pain PROBABLY RESULTS IN LITTLE TO NO DIFFERENCE in pain relief at 3-6 months (WMD -0.74 cm on a 10 cm VAS against a minimally important difference of 1.5 cm) or physical functioning (WMD 2.23 on the SF-36 100-point subscale against a 10-point MID), both moderate certainty; at 12 months pain was again probably little-to-no-different (WMD -0.73 cm). The measured effect is real but sits BELOW the threshold at which a patient would notice it.

MACI (autologous cultured chondrocytes on a porcine collagen membrane, Vericel; STN BL 125603) IS an FDA-LICENSED cell therapy - and its approved indication is narrow and specific: repair of symptomatic, single or multiple FULL-THICKNESS cartilage defects OF THE KNEE, with or without bone involvement, in adults. It is not approved for osteoarthritis. The existence of one licensed cartilage cell therapy for focal defects is the sharpest available way to show what a licensed 'regeneration' product actually looks like, and how far it is from an injection for a worn joint.

The SUMMIT randomized trial treated 144 patients (mean age 33.8, mean lesion 4.8 cm2) with at least one symptomatic focal cartilage defect (Outerbridge III/IV, >=3 cm2) of the femoral condyle or trochlea. Matrix-applied characterized autologous cultured chondrocytes (MACI) improved KOOS pain (37.0 to 82.5) and function significantly more than microfracture (pain 35.5 to 70.9) at 2 years, with histological and MRI assessment of the repair tissue. This is what a positive cartilage-repair trial looks like - in young patients with a discrete hole, not a worn joint.

A Bayesian network meta-analysis of 11 randomized trials (1353 patients) in hip osteoarthritis found that at 2-4 months and 6 months NO injectable - corticosteroid, hyaluronic acid or platelet-rich plasma - significantly outperformed a saline placebo injection for pain or function. Pooled change from baseline exceeded the minimal clinically important difference in every arm including placebo.
